About the Role
An exciting opportunity exists in our Positive Directions team which provides quality care and treatment to people living with HIV/AIDS (PLHIV) throughout Queensland.
We are seeking an experienced Clinical Nurse to join our compassionate and multidisciplinary team as a full time HIV Care Coordiantor. You will provide care and support utilizing a community based model and essentially provide chronic disease management to our clients. In return we can provide a supportive environment which will enhance and develop your HIV knowledge and skills.
Based at Camp Hill, the HIV Care Coordinator will work in collaboration with other members of the team with main responsibilities including:
- Effectively manage the coordination of care for clients;
- Deliver contemporary clinical care and services to clients and carers, safely and competently;
- Play a substantial, active and positive role in Program management and clinical administrative activities;
- Work in collaboration with other team members to ensure that client care needs, as identified and negotiated in care plans, are achieved.
You will be required to have:
- Current registration with the QNC;
- Understanding of and sensitivity to treatment, care support and information affecting people living with HIV/AIDS;
- Previous experience working in community health;
- Possession of current Queensland drivers licence;
- Eligibility to obtain a National Police Certificate;
- Knowledge of Total Quality Management and Continuous Improvement Processes;
- Ability to work effectively, respectfully and collegially in a team environment;
- Demonstrated health-related advocacy, referral and conflict resolution skills including ability to negotiate between individuals, communities and organizations whilst maintaining professional boundaries; and
- Proven ability to work independently whilst demonstrating behaviours consistent with Spiritus Values and Mission.
